Most Practices Never Build This.
The Few That Do Change Everything.

Dr. Beth Motley works with a small number of primary care practices each quarter to build fully operational lifestyle medicine programs: new revenue streams, better patient outcomes, and a practice you're proud of again. Applications are reviewed personally.

The Gap

You Already Know Lifestyle Medicine Works. Building It Into a Real Practice Is a Different Problem.

The Workflow Problem

Your schedule is already full. Adding a new service line without a clear system (and without disrupting your existing practice) feels like a problem you don't have time to solve.

The Revenue Problem

You've heard about shared medical appointments and lifestyle billing codes, but you're not sure what actually reimbursable looks like for your payer mix, or whether it pencils out.

The Roadmap Problem

The clinical evidence is settled. A practical, field-tested implementation guide built for real practices (not academic theory) is much harder to find.

Why This Works

She Built What She's Teaching.

Dr. Beth Motley didn't learn lifestyle medicine practice integration from a textbook. She built a functioning lifestyle medicine program inside one of South Carolina's largest health systems, designed and launched a Diabetes Reversal Shared Medical Appointment program at Prisma Health, and presented her practice integration framework at the American College of Lifestyle Medicine national conference, one of the field's most competitive stages.

When she works with your practice, you're getting a blueprint that has already been stress-tested in the real world. Billing figured out. Staff training built. Workflows mapped. Patient outcomes documented. Not a framework borrowed from a conference slide deck. A proven system, adapted for your specific practice, delivered by the physician who built it.

The Business Case

What This Actually Looks Like on a P&L

$80K–$125K

Estimated new annual revenue from a single twice-weekly shared medical appointment program (10 patients per group, standard reimbursement)

60–90 Days

Typical time from kickoff to first shared medical appointment running in your practice

6 Pillars

Evidence-based lifestyle medicine domains integrated into a complete, billable clinical program

Revenue projections are illustrative and based on typical insurance reimbursement rates for shared medical appointments. Actual results vary by payer mix, practice size, and implementation. Results discussed during discovery call.

Is This Right For You?

We Work With a Small Number of Practices Each Quarter. Here Is How We Decide.

This Is Designed For:

  • Primary care physicians (family medicine, internal medicine) who are already convinced lifestyle medicine works and are ready to build
  • Practices with a meaningful chronic disease patient panel (diabetes, hypertension, obesity, metabolic syndrome) who would benefit immediately from a structured program
  • Physicians who want to lead this clinically, not just delegate it to staff
  • Groups with at least one physician prepared to be the internal champion and see this through
  • Practices looking for a durable service line, not a one-time wellness event

This Is Probably Not the Right Fit If:

  • You are still researching whether lifestyle medicine produces real clinical results
  • Your practice has minimal chronic disease volume
  • You are looking for a fully automated program that requires no physician involvement
  • You need something operational in under 30 days
  • You want a generic template, not a custom-built program

Not sure which category you fall into? That is exactly what the discovery call is for. Apply below and Dr. Motley will personally review your intake before scheduling.

"So great to hear from a family doctor in solo practice utilizing resources from around her, rather than hearing from a team-based lifestyle medicine practice. This was great to get us headed in the right direction."

Common Questions

Before You Apply

Do you work with practices outside South Carolina?
Yes. The Blueprint Session and Practice Transformation Program are fully virtual and available to practices nationwide. The On-Site Intensive includes travel and is available anywhere in the contiguous United States.
What specialties is this designed for?
The program is built around primary care (family medicine and internal medicine). It can be adapted for endocrinology, cardiology, and other specialties managing significant chronic disease volume. If you're in a different specialty and curious whether this applies, mention it in your application and we'll be honest about fit.
Why isn't pricing listed for Tiers 2 and 3?
Because the right program depends on your practice size, payer mix, geographic location, and where you are in the implementation process. A 1-physician solo practice and a 10-physician group need different things, and pricing reflects that. We discuss investment on the discovery call after reviewing your application, so the conversation is useful from the first minute.
Do you help with billing and coding?
Yes, and this is one of the highest-leverage parts of every engagement. Understanding how to bill for shared medical appointments and lifestyle medicine services is where most practices leave significant money on the table. Every program includes a billing and coding guide specific to your payer mix and services.
How long until we see our first shared medical appointment running?
Most practices complete their first SMA within 60–90 days of starting The Practice Transformation Program. The On-Site Intensive is designed to compress that significantly; many practices schedule their first SMA before Dr. Motley's visit is over.
Can this work inside a large health system?
Yes. Dr. Motley built her own program inside Prisma Health and understands the administrative, compliance, and credentialing landscape that comes with health system work. The On-Site Intensive is typically the right fit for health system engagements. Mention your system affiliation in your application.
What if I'm not sure which program is right?
Start with the application. Dr. Motley reviews every submission personally and will recommend the right level of engagement based on what you share, including whether The Blueprint Session is the appropriate first step.
Apply

A Small Number of Spots. A High Standard for Both Sides.

Dr. Motley reviews every application personally. If there is a strong fit, you will hear back within 3 business days to schedule a no-obligation 30-minute discovery call. That call is not a sales pitch; it is a working conversation to determine whether your practice is ready and whether this is the right engagement for where you are.

If it is not the right time, she will tell you that too, and tell you what would need to be true before it is.
